best way to remove the hook from the fish's mouth '?

I really try to make the catch and release " but now 4 outa 5 times the fish died after i take the hook out. I'm wetting my hands before touching it, and I'm using a pair of needle nose pliers to catch the hook, it just seems like I am damaging the fish too bad for them to live. I use my pliers to consider beards, and not with acute hooks. it just seems like the hook is always in their throut too far to see sometimes. suggestions?
